MISHAP REPORTING SYSTEM
Auxiliarists' vehicles being used
to tow federal government-owned property
Recent legislative change has enabled the Coast Guard to provide
property damage coverage for damage to Auxiliarists' motorized vehicles that are
used to tow federal government-owned property. Essential elements to ensure
such coverage are 1) proper assignment to duty and 2) that Auxiliarists act
within the scope of such assignments. An additional pre-requisite is that the
Coast Guard must be made aware of the suitability of such vehicles for towing
federal government-owned property in advance of their usage by Auxiliarists
assigned to duty.
Summary of Mishap Reporting SystemThe Coast Guard Mishap Reporting System identifies accident trends within the Coast Guard, and allows for dissemination of lessons learned. Mishap Reports are non-attributional safety reports which cannot be used for administrative or legal actions. The Mishap Reporting System is not used for filing claims. To file a claim for damages or injuries, Auxiliarists would submit a report through the appropriate call-out authority.
Types of ActionAnytime there is damage or injury, whether it is a bent propeller or a twisted ankleWhen there are personal injuries to Auxiliarists, students or guests, whether underway on a boat on patrol, at an Auxiliary boating safety class or at an official social functionEven when no damage or injuries occur, but there are "close calls"

form to the Surface Stan Team (print out two copies)

The Auxiliarist in Charge should submit the Mishap Report. The person in charge may be a Coxswain, Aircraft Commander, Flotilla Commander, lead
Instructor or any Auxiliarist. Don't delay submission while attempting to determine who is in charge. If more than one report for an incident is received, reports will be consolidated.
The District Safety Officer will review the report, assign a case number, remove individual names and identification as appropriate, and forward the report to the District Director of Auxiliary. After further comments are added, the report will be forwarded to the Chief Directors office and disseminated as appropriate to the stations, sole state waters authorities and membership.
Remember, this information is for the benefit of the membership and public and is only used to track trends and educate members. The information will not be used for legal or administrative purposes.
